In addition, the fabrication of a final prosthesis is completed prior to the surgery. Computer-guided implant surgery is performed arthroscopically without requiring a large flap reflection. The process begins when a CAT scan is taken of the patient’s jaw bone.
This CAT scan allows for the generation of a three-dimensional model of the jaw bone that can then be used with virtual reality software to plan the implant placement without the presence of the patient. Teeth are fixed permanently as a fixed bridge on four implants following a personalized treatment plan, making All-on-4 an affordable, stable, and reliable solution for patients with missing teeth.
Many patients who may not have been candidates for implants in the past, due to complex bone grafting requirements and high costs of these procedures as well as the placement of large numbers of implants (generally 8-10), can often undergo this treatment due to the unique placement of 2 implants in the back of the jaw bone at a 45-degree angle, along with conventional implant placement (usually 2 implants) in the front of the mouth. The use of angulated implants in the back of the mouth overcomes the problem of bone height/volume deficiencies by avoiding enlarged sinuses, often without major bone grafting procedures.
Immediate dental implants and temporization, when properly indicated, has three main advantages: timing, biology and prosthetics. Treatment time and number of surgical procedures are reduced compared to a delayed approach.
From a biological standpoint, using a slow-resorbing material to fill the gap between the implant and the buccal plate enables predictably preserved bone volume. The provisional crown supports the gingival architecture and helps maintain the pre-existing positions of the gingival margin and interdental papillae.
From a prosthetics and cosmetic dentistry perspective, placing an implant retained provisional crown on the day of surgery simplifies the temporization in the esthetic area, allowing the patient to leave the office on the same day with a fixed provisional.
The success of this procedure relies on three basic principles: proper indication, atraumatic extraction and sufficient primary stability of the implant. The latter depends widely on the choice of the implant design and drilling protocol, which should be considered for greater primary stability.

Traditional implants are often used to replace one tooth, several teeth, or a smaller section of missing teeth. In some cases, one implant supports one crown. In other cases, two or more implants support a dental bridge. For full-mouth treatment, traditional implant plans may involve more implants, more appointments, and a longer healing period before the final teeth are placed.
All-on-4 dental implants are designed for patients who need to replace a full upper or lower arch of teeth. Instead of placing an implant for every missing tooth, four implants are placed in key areas of the jaw to support a fixed full-arch restoration. The back implants are often placed at an angle to make better use of available bone and help avoid more complex bone grafting in suitable cases.
All-On-Four dental implants are fixed in place and need to be treated like permanent teeth. They need proper care to keep the gums, bone, and any existing teeth healthy.
Unlike removable dentures, All-On-4 teeth are a permanent solution and are not taken out at night. You will brush them while they are in your mouth, just like natural teeth. However, you also need to clean under the bridge and around the implant posts where the restoration meets the gums.
Regular dental checkups are still needed after All-on-4 treatment. During these visits, your dental team can check your bite, clean areas that are hard to reach at home, examine the gums, and make sure the implants and restoration are stable.
