Financial Policy

Thank you for selecting our specialty practice for your periodontal and implant treatments. We have been committed to providing our patients with the highest standard of care since 2001.

Our specialist doctors, Dr. Noroozi, Dr. Pauletto, Dr. Zaeimdar and staff are proud to be a team whose primary mission is to deliver the finest and most comprehensive periodontal services available today. We are concerned about your dental care and want to ensure you that it is performed in the most responsible manner. In order to assist you with the investment in your dental health, we have outlined our payment policy. Please understand that payment for services rendered is part of your treatment. 

Emergency/Single-Visit Payments

For single-visit procedures, our policy is that payment is due in full at the time of service. For your convenience we accept cash, bank draft, debit and credit card payment (MasterCard, Visa, American Express).

Multiple Visit Payments

For multiple visit treatment plans you will be provided with a written treatment plan and estimate of cost which will be reviewed with you prior to beginning treatment. Payment will be due in full at the time each step of the treatment is completed.

Outside Financing

For those who would prefer an extended payment plan; we would be happy to assist you in making financing arrangements with Dentalcard, a convenient alternative that lets you pay for your dental care over time. You can learn more about Dentalcard on their website.

Insurance

In the best interest of your dental health, your treatment plan will be based on the diagnosis made by your specialist doctor and you will be informed of the estimated cost of your treatment. Even if you have dental insurance, your treatment will not be dictated by available benefits. Our specialists will offer a treatment plan that will best suit your needs. This treatment plan may or may not fall within the parameters of your dental plan.

As a service to our patients with dental benefit plans, we will submit the necessary claim forms, receipts, and other information to your insurance company. This can be done electronically in most cases. Your insurance company will reimburse you directly for the portion of payment to which you are entitled, as outlined in your insurance plan. We do not collect payments from insurance companies; however, upon request, we can obtain a pre-determination of dental benefits prior to treatment. Please be advised that a pre-determination is not a guarantee of payment, and that you are directly responsible for the payment of all fees. We do ask your participation however, to expedite this process, by phoning your insurance carrier directly, if you do not receive payment for the treatments already performed or if you are still waiting for pre-approval for upcoming treatments for a period longer than 4 weeks. We also encourage you to contact our office immediately if you have been waiting for a long period of time and if you have any questions or concerns regarding your current coverage.

We also want you to be aware that we require deposits towards reserving your surgery appointment with our specialists and for long appointments with our hygienists. This deposit will of course be applied towards treatments. We require three business day notice to cancel a scheduled appointment. This is done strictly to insure the best treatment outcome for our patients, and to ensure that each patient is given ample time to prepare for the surgery, with regards to time off work and also pre and post operative instructions which will be given to you at the time of scheduling these appointments.
